Medical Transportation provides transportation to and from medical appointments covered by OHIP and/or Non-Insured Health Benefits for Six Nations Band Members (living on and off reserve), New Credit Band Members and other First Nations.

Intake/Referral Process:

Six Nations Band Members: if you require NIHB Medical Transportation, please call 519-445-0410 at least 5 days prior to your appointment.

New Credit and other First Nations: medical transportation requires prior approval, so requests must be made at least 2 weeks prior to your appointment.

When you call, please have the following information available:

  • Your full name

  • Your date of birth

  • Your complete address and telephone number

  • Your 10-digit status card registration number

  • Your doctor's name and the area of specialty

  • Date and time of your appointment

  • Address of appointment

  • Any other medical transportation needs (i.e. wheelchair; escort)
