As the population of our senior community (aged 65-85), the frail and disabled citizens increases, the need for more public home and community care is required.
We must convey a positive attitude about the care of this target population as many may experience difficulties when faced with performing day-to-day tasks. Also, the likelihood of illness and disabilities increase with age
The Six Nations Home & Community Care Program, in partnership with community agencies and members, is committed to providing culturally appropriate service in home for individuals from newborns to seniors within the Grand River Territory.
We are respectful of individual choice in the direction of care.
The Home and Community Care Program is assertive and proactive and as a member of the Six Nations Health team, is dedicated to meeting the challenges of the changing paradigm with community care in order to meet the needs of the Six Nations target population.
